Description:
Social determinants of health are the conditions in which people are born, live, work, learn and play. Addressing social determinants of health is critical for improving health outcomes and reducing health disparities in our community, yet there are real-world challenges in effecting interventions within our existing healthcare and legal systems. This panel seeks to examine how: 1) social determinants of health are being addressed in our communities; 2) highlight success stories and challenges; and 3) identify some possible models and strategies for future improvements such as medical-legal partnerships and collaborative networks with community-based organizations (CBOs). The panel will also discuss the utility of partnerships between CBOs, healthcare providers, and lawyers to address the social drivers of health, the challenges to their implementation, and policy recommendations to improve future health outcomes.
